Same Day Instructions for: Lost/Stolen In order for us to process your passport for you, you must first go to a local acceptance location which will either be a special post office or county clerk's office. Make sure you take sufficient identification, such as a driver's license with you. You will need to show the items listed below, which will then be put into a government sealed envelope. Make sure you get the envelope back from the clerk, and do not open it. Follow these instructions if you are leaving the country within 4 business days. Requirements 1) DSP-11 brown passport application 2) DSP-64 gray lost/stolen form 3) Original Certified Birth Certificate, or  orignal naturalization certificate, or a previous passport. 4) Two identical passport photographs. 5) Valid Picture Driver's License issued more than 6 months ago. If your Driver's License has been issued or renewed in the last 6 months, you will need to provide at least 3 personal documents. 6) 2 copies of Letter of Authorization (print out and fill in blanks) 6a ) Both parents must appear with the child at the acceptance agent when obtaining the sealed envelope. If both parents cannot be present then a Written Statement of Consent must be provided to the acceptant agent. 7) 1 check made out to Department of State for $115.00 for adults and $100.00 for children and another check for $30.00 for the post office/county clerks fee. 8) Lost/Stolen Statement 9) You must have proof of departure, which will be ONE of the following:      a) 2 copies of airline tickets or      b) 2 copies of travel itinerary or      c) 2 copies of company letter of expedite (click for sample) 10) You must fax a copy of the Instructions for Same/Next Day Service of Passports to us so we know that you are sending a passport to us.We then reserve a space for your passport application with the passport office. Please print and fill out the Instructions for Same/Next Day Service of Passports and attach a copy to your sealed goverment envelope.Also include a copy inside your Fed Ex Priority Overnight envelope. In the Federal Express envelope there should be 4 things which include: 1. Sealed Envelope 2. Letter of Authorization 3. 1 copy of your proof of departure 4. A copy of the Instructions for Same/Next Day Service of Passports form.
